Accelerated Healing for Chronic Diabetic Wounds

Our Mobile MD provides:

  1. Traditional Wound Care
  2. Biologic Wound Care
  3. Weekly wound evaluation and treatment
  4. Monthly skin assessment
  5. Monitoring and managing infections
  6. Turnaround time is 24-48 hrs after receiving the referral and 24-48 hours to send documentation.
  7. Providers go to the house once a week for the treatment and assessment.
  8. In case of dressing changes, the home health covers for that or the family of the patient. Dressing changes may be done 1x, 2x, 3x a week that will be ordered by our MD/NP/PA.
  9. A wound vac may be placed at home on the patient’s wound or a patient may have wound vac coming home from the hospital. The wound vac is a medical device that uses negative pressure to remove fluid, debris, and bacteria from wounds, promoting healing.

  1. Stage 2,3,4, and unstageable wounds
  2. Patient that has Medicare Part B
  3. Wound care order
  4. Wound Pictures would be great to help speed up the intake process of the referral
  5. Face Sheet
  6. History and Physical
  7. Medication Administration Record
  8. Progress notes
  9. Lab tests
